I'm afraid im going to die?

for about 4 month now iv been feeling an ache in my left testicle, i feel a lump but it is right next to or on the epididymis so im unsure if its a lump or swelling, im 15 years old and i find it so hard to tell my parents. i am now getting lower back pains and chest pain, and i can feel a lump gland type thing in my jaw, are these linked or am i paranoid? im quite an athletic kid so its all confusing to me. please help me and dont just tell me to see a doctor cos i know that, i just want to be reassured? :/
